Healthiest Greenfield 2022

The Healthiest Greenfield 2022 Community Health Improvement Plan (CHIP) was developed using a community-driven process to outline the strategic goals and objectives for action to improve our community's health. This plan will be in place for the next four years and will help to guide the efforts of the Healthiest Greenfield Coalition.

The Healthiest Greenfield Coalition is a robust group made up of community organizations, residents, and other important partners that are passionate about health and wellness. This coalition uses the CHIP to guide efforts to bring programs, services, and other resources to our community that focus on these three priority areas:

  1. Behavioral Health
  2. Healthy Aging
  3. Nutrition and Physical Activity

Healthiest Greenfield Coalition welcomes all community members to join and be a part of improving our community’s health! As a member, we invite you to participate in Quarterly Meetings, providing your email to stay in touch with important health updates and volunteer opportunities and to learn more about current or upcoming community events and programs.
