Kids Connection Registration 

2025-2026 SCHOOL YEAR

Registration for the 2025-2026 School Year is now open. Form links are below and you must complete a new form for each child each school year.

PAYMENTS

Payments will be made in advance. The first payment will be due by August 20th. Registrations received after August 20th will have the payment processed upon receipt.

  • Cash or Check Payment: Due at the Parks & Recreation office by 5:00 PM on the 20th of each month to pay for the following month. If the 20th falls on a weekend, payment is due the business day prior to the 20th. If we do not receive payment, a late fee of $15.00 per family will be charged. Any checks returned for insufficient funds will be charged a $25.00 service fee.
  • Auto-Pay: Credit card only (Visa, MasterCard, Discover, or American Express). Your credit card will automatically be charged the 20th of each month to pay for the following month. If the 20th falls on a weekend, your card will be charged the business day prior to the 20th.

REGISTRATION FORMS

Register by downloading the form and returning to the Parks & Recreation office or complete our NEW online form. It may look a bit different but we hope you find it easier to complete. Please call the office at (414) 329-5370 with any issues in completing the online form.

NEW PROCEDURE FOR THE 2025-26 SCHOOL YEAR for Families splitting payments for Kids Connection: Each payee must submit a registration form and schedule for each child enrolled.

If you need to make a change to your schedule or you'd like to cancel out of Kids Connection, you must submit our Change/Cancellation form:

MEDICATION FORMS

Does your child need medication while at our program? If so, please fill out the proper form and hand in along with your registration form. Emergency medication such as Epi-pens, will need the prescription medication form completed. Any questions, please contact us.
